2009/10 ENROLLMENT INFORMATION

Payment of $50 Application Fee
Complete and submit Registration Application and applicable fees
Incoming Middle School Students must complete and submit both recommendation forms:
- Middle School Recommendation Form 1
- Middle School Recommendation Form 2
Fill out a Financial Agreement
Submit a copy of previous year's report card and standardized test scores
Schedule testing date
Schedule meeting with Principal
Kindergarten and incoming 1st grade: submit completed Report of Health Examination for School Entry form and immunization record
Attend new parent orientation and Back-to-School Night

Application is to be completed and returned to the school office with a $125 non-refundable Registration Fee
A separate book and materials fee of $50 is due upon acceptance
At that time you will be given forms to be completed and returned by the first day of your child’s attendance
Tuition is due on the first of each month
